Consider converting it to an editable PDF file. Send us a list of your questions, or your current questionnaire. We can work with a range of formats, including a scanned image, a ‘flat’ PDF, a Word document, or Excel spreadsheet.

We can supply a PDF file with format checks, such as email address verification. The PDF can include URL or email links and more.
